Hi David,
As per Install macOS 10.14 Mojave on Mac Pro (Mid 2010) and Mac Pro (Mid 2012) - Apple Support, the Quadro 4000 isn’t a card that “works with” Mojave, so I don’t know that there’s anything we can do about your problem.  You may have to roll back.

Presumably because the graphics driver is not working correctly.

Has anyone succeeded in resolving this? Or do I need to roll back to 10.13? A quick scan of the Mac/nVidia Forums suggests that CUDA support may not be forthcoming anytime soon, but I thought I would check if anyone has resolved this before going through the hassle of downgrading.
